Foot Pain
Cycling is fun. Foot pain should not be part of it.
Especially the front foot can get painful as you spend time on the bike.
The reason is that all pedal pressure transfers onto a small area having delicate nerves and tendons.
Wheather this is diagnosed as Metatarsalgia, Morton's Neuroma, or hot-spots, the solution may often be the same: 1) Move and 2) Distribute the pedal pressure.
Classic vs Rearward Cleat Position
Historically, only one cleat location was feasible: Under the ball of the foot. This was the only place where old, soft cycling shoes could transfer any power.
Strangely, despite modern rigid soles, cycling shoes still locate cleats as in the old days. For many, this is not optimal.
Those who suffer from foot pain, Achilles tendon pain, heel drop, or tired calves would benefit from a more rearward cleat position.
Cleat Position and Foot Comfort
For foot comfort, an efficient remedy is to move the pedal's pressure away from the painful area.
Often a 10-24mm rearward cleat move is all it takes.
Unfortunately, no cycling shoes offer that much displacement.
